Photo Waiver

FLORIDA HARDBALLERS BASEBALL ACADEMY, LLC

MODEL RELEASE AND WAIVER

This section must be completed by an individual participant over the age of 18 years old.

I hereby grant permission to Florida Hardballers Baseball Academy, LLC (“FLHB”) to photograph, record, and use the below-named Player’s name, image, voice, and likeness for reasonable purposes related to FLHB and its baseball programs.

Permitted uses may include, but are not limited to, FLHB’s website, social media accounts, team and tournament promotions, organizational marketing materials, newsletters, team communications, fundraising materials, advertisements, and other activities intended to promote or document FLHB, its teams, players, events, and programs.

I understand that these materials may be publicly accessible and may be reproduced, distributed, shared, or reposted in connection with FLHB activities. I acknowledge that neither the Player nor I will receive compensation for the authorized use of these materials, and I waive any right to such compensation.

I further understand that FLHB may share these materials with sponsors, media outlets, tournament organizers, and other third parties for purposes consistent with FLHB’s programs and promotional activities, without advance notice or additional permission. FLHB retains the right to use and reproduce authorized materials indefinitely for these purposes.
